What is the Adult Social Prescribing Scheme?
The Adult Social Prescribing Scheme (SPS) enables practice staff to refer patients who are presenting in General Practice with social, practical and mild to moderate emotional needs to a Wellbeing Link Worker who is able to provide patients with non-clinical and practical support. Patients can also refer themselves to receive this support.
The SPS improves individual health outcomes by connecting people to activities, groups, and services to meet the practical, social and emotional needs that affect their health and wellbeing.
The Role of the Wellbeing Link Worker
Our team of Wellbeing Link Workers deliver personalised care to people and this commonly consists of (please note this list is not exhaustive of all the types of support available via the SPS):
- Comprehensive assessment of your social, practical and emotional needs.
- Evaluation of the link between your health and current social, practical and emotional needs.
- Providing information about educational tools/resources that will help you to improve health outcomes.
- Creating appropriate and beneficial links to activities, groups and non-clinical services that will help you to improve health outcomes.
- Support in finding available support groups for specific health conditions.
